Evidence-gate card
Penvara Professional AI
Fictional company, real review pattern.
Penvara Professional AI is a generative copilot used inside professional-services firms, where a person reviews and approves what it drafts. Its technology and cyber cover is up for renewal, and the underwriter's questions now include AI governance.
Under renewal time pressure, the founder assembles answers from scattered sources. The underwriter cannot easily tell which evidence came from a system at the time and which is a later description, so the file is hard to weigh.
Custara prepared a single renewal record, showing model change, oversight and data-source evidence in a format the underwriter could weigh quickly.

Featured item (expanded): the record shows, per item, how it was captured, whether from a system at the time, documented at the time, reconstructed, or stated by the operator. The underwriter can see that the oversight settings were captured at the time, while one attestation is stated, and weigh them accordingly. Kept intact: record reference retained with dated capture details.
Fictional record. Status reflects record completeness, not governance judgement. Not a score, an assessment or a certification.
Evaluation history still needs source, and one liability item is missing. The record shows that without turning it into a score signal.
Shown, not hidden. No score state.
A controlled renewal pack in the underwriter's format, with only the relevant evidence categories included.
The same record can answer procurement review and next year's renewal after it is updated.
Custara made the evidence comparable and showed how each item was known. The underwriter weighed it and priced the risk.
